Contact the location site to be aware of any specific rules or requirements regarding notarization on-site
Verifying the identity of your loved one to be sure they are who they say they are (see IDs)
Ensure that the signer is competent of sound mind and understands the nature and consequences of the document they are signing
If notary suspects any coercion of the signer, the notary will ask that the room be cleared of all non-medical personnel
Be mindful of the staffing schedule so the appointment doesn’t interfere with shift changes
If signer has the inability to sign their name, please advise us so we can discuss an alternate option called ‘Signature by mark’
Note: Keep in mind for any signing that takes place at a skilled nursing facility (a nursing home and/or long-term care facility), it’s required to have a patient advocate called an Ombudsman present to witness the notarization process as they act as an advocate for the resident to be sure they are alert and aware of what is going on.
